What is the Bundled Care for Post Hip and Knee Replacement Program?
Bundled care is a service delivery and funding model that is designed to promote greater integration in health care delivery, drive high-quality, efficient care and improve patient outcomes and experience.
The Ontario Ministry of Health has introduced a bundled funding payment model for integrated acute to post-acute (including rehab) pathways for multiple procedures or conditions. The bundled care model for unilateral hip or knee replacement patients has moved into full scale implementation with all 56 hospitals across Ontario that perform these surgeries. The model is based on having one single payment made to the bundle holder (usually a hospital site) who then establishes partnerships to provide needed follow up care, including rehabilitation.
What does this mean for you?
SPINEgroup has entered into partnerships with all hospitals serving the Greater Toronto and York Region areas and is able to provide post surgical rehabilitation for unilateral hip and knee replacement patients.
If you are planning to receive a unilateral hip or knee replacement, the case coordinating at your hospital will provide you with a Bundled Care Package upon discharge. You are expected to contact the clinic and book an appointment to start your rehabilitation within 2 weeks post discharge. You will be required to bring your Bundled Care Package for eligibility when you enrol for your rehab with SPINEgroup. We will bill for your rehab directly to the referring hospital.
